Informed Consent Form
The latest Informed Consent Form is downloadable Here- please note these forms must be completed by all participants of your Explorer Days in order for them and you to be covered by the YNC Liability Insurance. These forms must be sent to the office twice yearly where they will be stored until the youngest child on the field trip turns 18. (Aren't you glad the office is keeping them!).
Incident Report Form
In the unlikely event that an accident or incident occurs during an Explorer Day or other Club Event make safety of the injured and any others present your immediate concern. Then record the details of the incident as soon as possible while the event is still fresh in your mind, using the Incident Report Form below.
